An athlete may contact the Head Coach for the team Off-Season Waiver form. The waiver form will allow the athlete to have direct training and competitive contact with team coaches and/or team sponsors during the off-season. The season dates are defined under athlete eligibility and will be noted on the off-season waiver form at the time of signature. The intent of the waiver policy is for full disclosure and notification to protect the athlete, coaches, and the team against violation of MSHSL and school policies. The waiver form must be filled out by the athlete and signed by the parent/guardian. The submitted form will be reviewed by the Head Coach and submitted to the Board of Directors for approval. Notification will be sent to the Apple Valley High School Team Advisor, the Minnesota State High School Clay Target League, and the USA High School Clay Target League.
Participation in any off-season activities covered under the waiver form are NOT considered team events and are NOT covered by the club’s insurance policy. As these are NOT team events, the team uniform jersey cannot be worn during events covered under this waiver.
The athlete must have the signed and approved team off-season waiver document in possession during off-season activities in which the document is required.
